Most rehab programs treat you like a protocol, not a person. You get a generic exercise list with no explanation of why, no progression logic, and no one checking whether it's actually working for you.
My approach is different. As both a licensed physical therapist and a strength & conditioning specialist, I bridge the gap between rehabilitation and performance — so you don't just recover, you come back stronger than before.
Every protocol is grounded in current sports medicine research, adapted to your specific presentation.
Your load, your pace, your goals. I review your progress and adjust your program every step of the way.
We don't stop at "pain-free." We train you to be athletic, resilient, and ready for your sport.
Before any program begins, we understand your history, your body, and your specific goals — not just your diagnosis.
Built for you, not copied from a template.
Regular check-ins, adjustments, and progression as you improve.
My first program is open now. Additional programs are in development and will be launching over the coming months.

"As a sprinter, Achilles issues are terrifying — you never know if you'll come back the same. Shana understood the demands of sprinting from day one and built a program around that, not just generic tendon rehab. She progressed the load at exactly the right pace and I always knew what we were working toward. I'm back to full training and my tendon has never felt more solid."
"I finished my ankle fracture rehab but still felt nervous about my ski trip coming up. Shana put together a specific ski prep program that focused on strength, stability and confidence — exactly what I needed. The Optimal app made it so easy to follow: videos for every exercise, everything organized, no guesswork. I went into that trip feeling better prepared than I thought possible."
"ACL and meniscus at the same time — I was dreading the rehab. But following Shana's program through the Optimal app made it manageable. Every exercise had clear videos, and getting feedback from Shana online meant I never felt like I was guessing. By 3–4 months post-op I was confident enough to train more independently, which was always the goal. I check in once or twice a month now and I'm in better shape than before the injury."
PT · MSc Strength & Conditioning · Sports Rehabilitation Specialist
I'm a licensed physical therapist with a master's degree in Strength and Conditioning, specializing in sports rehabilitation and online performance coaching. I work with active people and athletes who want a clinical expert in their corner — not just a program.
My practice is built on a simple belief: rehabilitation and performance training aren't two separate things. When done right, they're the same conversation.
My path to online rehabilitation coaching started long before I had any formal credentials. As an athlete myself, I experienced firsthand the frustration of inadequate rehabilitation — programs that left me with exercises but no context, numbers but no nuance, and a return-to-sport date that had nothing to do with how I was actually moving.
That experience shaped everything about the way I practice. I pursued physical therapy because I wanted to be the clinician I'd needed — someone who understood both the science of tissue healing and the demands of athletic performance. My master's in Strength and Conditioning wasn't an afterthought; it was a deliberate extension of how I think about the body: always within the context of what it needs to do.
My clinical background spans orthopedic and sports settings, where I've treated athletes across a wide range of sports and skill levels — from weekend runners to competitive athletes returning from surgical reconstruction. That experience taught me that no two recoveries are the same, even when the diagnosis is identical.
Online coaching was a natural evolution. It removes the geographic barrier between athlete and expert, and when it's done well — with real communication, individualized programming, and ongoing clinical oversight — it can be more thorough than a 30-minute in-person appointment.
The best sports PT for you might not live in your city. Online rehabilitation closes that gap. It also creates something often missing from clinic-based care: a relationship built on continuity, where I see your program across weeks and months, not just one session at a time.
My clients do their sessions independently, in their own environment, with equipment that mirrors what they'll use in daily life and sport. They send me video check-ins, I review and adjust their programming, and we communicate regularly — so nothing falls through the cracks between appointments.
Evidence-based templates are a starting point. I individualize everything based on your history, presentation, and goals.
I use objective criteria — strength ratios, movement quality, load tolerance — not arbitrary timelines, to guide your return to sport.
Understanding your condition and your program is part of the treatment. Informed athletes recover better and stay healthier.
We don't stop at pain-free. My goal is for you to be stronger, more resilient, and more capable than before your injury.

Limited SpotsA progressive, evidence-based loading program for athletes with midportion or insertional Achilles tendinopathy — designed to eliminate pain, rebuild tissue capacity, and return you to running, jumping, and sport with confidence.
This isn't a generic exercise sheet. It's a clinical program, individualized to your load history, your pain pattern, and your performance goals — with ongoing oversight from a sports PT throughout.
Achilles tendinopathy is a painful overuse condition affecting the Achilles tendon — the large tendon connecting your calf muscles to your heel. It's one of the most common running and jumping injuries, affecting recreational athletes and competitive sports people alike.
Unlike a tendon tear, tendinopathy involves a breakdown in the internal structure of the tendon due to accumulated load that exceeds the tendon's capacity to adapt. This is important because it means the solution isn't rest — it's progressive, well-managed loading that helps the tendon rebuild its structural integrity.
There are two main types: midportion tendinopathy (pain in the middle of the tendon, 2–6 cm above the heel) and insertional tendinopathy (pain right where the tendon meets the heel bone). Both respond well to evidence-based loading programs, but the exercise prescription differs — which is one reason why a generic program often fails.
The good news: Achilles tendinopathy, even when it's been going on for months or years, responds well to structured progressive loading. Research is clear that exercise-based treatment is the gold standard — but the program needs to be appropriate for your tendon's current capacity, your pain pattern, and your training demands.
Note: the final phase includes sport-specific jumping and change-of-direction work. If your goal is simply pain-free daily activity or recreational exercise, we work to that target instead — the program adapts to you.
Not sure? Apply for a discovery call and we'll figure it out together. If this program isn't right for you, I'll tell you honestly.

The science of Achilles rehab is well-established. The art is knowing how to individualize the progression — that's where clinical oversight makes all the difference.
We begin with a full clinical intake to establish your pain pattern, baseline strength, and current load tolerance. Isometric exercises provide immediate pain relief and begin to stimulate tendon adaptation without aggravating reactive tissue.
The cornerstone of evidence-based Achilles rehab. Heavy slow resistance training rebuilds the structural integrity of the tendon and develops the calf strength required for running and jumping. Intensity and volume are progressed systematically based on your weekly response.
Once the tendon has sufficient strength and load tolerance, we introduce faster, more dynamic exercises that train the tendon's ability to store and release energy — the mechanism required for running and jumping. Graded return to running begins in this phase.
The final phase bridges the gap between running and your specific sport demands. We reintroduce cutting, jumping, change of direction, and sport-specific movements, building toward full competitive return. Objective criteria — not calendar dates — determine readiness.
Most people see significant improvement within 8–12 weeks, with full return to sport typically at 12–16 weeks. Chronic cases or insertional tendinopathy may take longer. The timeline depends on your current pain level, how long you've had symptoms, and how consistently you complete the program. I'll give you a realistic expectation after your initial assessment.
It depends on your current pain level and how reactive your tendon is. Many people can continue modified running from the start; others need a brief reduction in impact load before reintroducing it gradually. This is exactly the kind of individualized decision that separates this program from generic advice. We'll establish your personal "pain rules" in week one.
Eccentric protocols (like the Alfredson protocol) are a component of Achilles rehab, but alone they're often insufficient — especially for insertional tendinopathy, where eccentric exercises can actually aggravate symptoms. Current evidence supports a broader heavy slow resistance approach, combined with appropriate load management, hip and knee strengthening, and progressive plyometrics. I also review your response weekly and adjust — which a static protocol can't do.
For most of the program, you need a step/stair edge and a way to add weight (a backpack with books works fine early on, a dumbbell or resistance band later). In later phases, access to a gym with a leg press is helpful but not essential. I'll always provide alternatives if you have limited equipment.
After your initial video consultation, I build your individualized program and deliver it via a coaching app — with video demonstrations, sets, reps, and explanations for everything. Each week, you send me a short video of your exercises and a pain/response update. I review these and respond with written feedback and any program adjustments. You can message me with questions anytime. It's more thorough than a 30-minute in-person appointment where the PT watches you do 2 reps and sends you home with a printout.
Many people worry their situation is too complex for online care. In practice, most Achilles tendinopathy cases — including those who've had cortisone injections, long-standing chronic presentations, or previous failed programs — are very well suited to online rehabilitation. Apply for a discovery call and we'll assess this together. If in-person care is genuinely what you need, I'll tell you.
This program is delivered through the Optimal app, which gives you access to your personalized workouts and exercise videos. Timelines and outcomes depend on individual factors including injury severity, consistency, and baseline fitness. Results described in testimonials reflect individual experiences and are not guaranteed. This program is not a substitute for in-person medical assessment where clinically indicated. If you are unsure whether online rehabilitation is appropriate for your situation, please reach out before applying.

"ACL and meniscus at the same time — I was dreading the rehab. But following Shana's program through the Optimal app made it manageable from day one. By 3–4 months post-op I was confident enough to train more independently, which was always the goal. I check in once or twice a month now and I'm honestly in better shape than before the injury."
"As a sprinter, Achilles issues are terrifying — you never know if you'll come back the same. "I finished my ankle fracture rehab but still felt nervous about my ski trip. Shana built a specific ski prep program for me — strength, stability, and confidence on the slopes. The Optimal app made it so easy to follow: videos for every exercise, everything organized, no guesswork. I went into that trip feeling more prepared than I thought possible."
"The communication throughout was excellent. Quick replies, clear progressions, and Shana always explained the reasoning behind what I was doing. I never felt like I was just following a plan blindly — I understood my injury and my recovery, which made the whole process so much less stressful."
"I was skeptical that online rehab could be as effective as in-person. It was. Shana's S&C background means she doesn't just treat the injury — she thinks about your whole body and what it needs to do. I came away with a completely different understanding of how to train sustainably."
"I was nervous to invest in something I hadn't tried before. Looking back, it was the best decision I made for my health in years. The program paid for itself in avoided physio appointments, race entry fees I didn't have to forfeit, and just the ability to do what I love again."
"As a physio myself, I had high standards for what I wanted from online rehab. Shana exceeded them. The clinical reasoning behind her programming is sound, the communication is excellent, and she genuinely individualizes — nothing felt like a template."
Everything you need to know about online rehabilitation coaching with Shana — how it works, what to expect, and whether it's right for you.
After your initial video consultation, I design your individualized program and deliver it through the Optimal app — a coaching platform that gives you organized workout sessions and exercise demonstration videos, all in one place. You follow your program independently at a time that suits you. Throughout the program, you have messaging access to ask questions (frequency depends on your plan), and we have a monthly video call together. I review your progress and adjust the program based on how you're responding. Think of it as having a sports PT available throughout the week, not just for one appointment.
For musculoskeletal and sports injuries that don't require hands-on treatment (which is the majority of rehab once acute pain is managed), online rehabilitation can be equally or more effective — because it's individualized, continuous, and you're doing the exercises properly in your own environment. The growing evidence base for telehealth in physiotherapy is strong. That said, I'll always be honest if I think your case needs in-person care.
A smartphone or computer with a camera is all you need. I use a coaching platform that works on any device. For video consultations, we use a standard video call (Zoom or similar). Being able to film your exercises with your phone is helpful — setting it up on a chair or windowsill works perfectly.
Setbacks are a normal part of rehabilitation. If your symptoms increase significantly, you message me immediately and we adjust. This is exactly why having clinical oversight throughout matters — a static program can't respond to what's happening in your body. I build pain monitoring frameworks into every program so we catch and respond to flare-ups before they derail progress.
Start by applying for a discovery call. You'll briefly describe your injury, history, and goals, and we'll talk through whether one of my programs is a good fit — and if so, which one. If you're not sure of your diagnosis, I can often help clarify based on your symptoms and history, and recommend further investigation if needed.
Not necessarily. Many people come to me with a suspected injury and a history of symptoms but no formal imaging or diagnosis. As a licensed PT, I can conduct a clinical assessment and help determine the most likely diagnosis. In some cases I'll recommend you get imaging or see a specialist first, and we'll work around that.
Most programs require 3–4 training sessions per week, each lasting 30–60 minutes depending on the phase. The weekly check-in video and update takes about 10 minutes. The time investment is meaningful — but so are the results. I'm also realistic: if your schedule changes, we adapt.
In most cases, yes — though the specifics depend on your injury and how reactive it is. I integrate your rehab with your current training load wherever possible. Part of my role is helping you manage the total stress on your body, not just prescribing exercises in isolation.
I'm based in Israel (GMT+3) and work with clients internationally — there are no geographic limits to online rehabilitation. The only requirements are a shared language (English) and reliable internet for video consultations. Video calls are scheduled to suit your time zone.
Currently my programs are self-pay. Some clients are able to claim reimbursement from their health insurance for physiotherapy services — I can provide documentation to support this, but I recommend checking your policy's terms for telehealth/online physiotherapy coverage directly with your insurer.
You leave with a full understanding of your injury, your rehab process, and a maintenance program to keep you healthy long-term. Many clients continue with ongoing performance coaching after their rehabilitation is complete. You're also always welcome to reach out if something comes up down the line.
